To ensure your application is processed efficiently and accurately, please follow these tips when preparing and uploading your documentation.

1. Accepted File Formats and File Sizes for Attachments

The accepted formats for uploading any attachments are:

  • Portable Document Format File (.pdf)
  • Microsoft Word Document (.doc)
  • Microsoft Word Open XML Document (.docx)

The maximum file size is 1000 MB. The smaller the file size, the better.

2. Follow the Recommended Document Order

Complete Profile Form:

  • Name Change Documentation (if applicable)
  • Ahpra licence notations, conditions, undertaking, or reprimands (if applicable)
  • Membership Evidence (if a member)'

Select Credentials - Application Form:

  • Qualifications
  • Employment Verification
  • CV
  • CPD Record
  • References

3. Scanning and File Preparation Guidelines

  • Scan each document separately. Do not combine multiple documents into a single file. This makes it easier for reviewers to locate and assess each item.
  • Ensure all scans are clear and legible, with no cropping or distortion.
  • Use landscape or portrait orientation appropriately—documents should be scanned in the view that best preserves readability.
  • Only use the 'Additional Upload Space' if necessary. Prioritise uploading documents in the designated sections first.
  • All documents must be certified where required. Refer to page 6 of the C4N Application Guidelines 6Oct2016 for certification standards.

4. Avoid Common Pitfalls

  • Do not upload documents out of order—this can delay the review process.
  • Avoid submitting incomplete or uncertified documentation, which may pause your application and extend review timeframes beyond the standard 8 weeks.
  • Ensure referees are not relatives or close personal contacts, and that they are familiar with your credentialing application.
